It is often necessary to prescribe how much gluten to eat. I Ask patients to eat increasing amounts of gluten, starting from a cracker building up to 4 slices of whole bread a day and to keep taking that so long as they do not get too ill, but should get definite symptoms. When I left it up to the patient, it seems they ate less gluten and more sporadically. It is sometimes hard to wait to persuade the patient to wait long enough for damage to occur. While the standard 4 week challenge will pick up most it will not pick up all patients. In those patients who remain asymptomatic I continue to wait and monitor the antibodies as well as the development of symptoms and then biopsy. I also find it useful to review the original biopsy material and other primary information on which the original diagnosis was based. Sampling problems and interpretation are also issues.
There a few people I will not challenge, People who are so nutritionally deplete that might not tolerate the challenge, people who give a history of anaphylactic response that could be life threatening and possibly those with a history of sever neurologic problems.

Hi,
I recently tested positive for the celiac serology test. After researching about celiac I was angry that my Dr just told me to have a gluten free diet without any further testing. I don't really want to be gluten free and would like confirmation. I am now scheduled for a gastroscopy in about a week and a half, but have been eating gluten free for the past couple of weeks on Dr's recommendations. When I asked him for referral he did not even mention that I would maybe need to up my gluten intake (gluten challenge) for accuracy of the biopsy. No one has even gone through what the gastroscopy involves and I am really frustrated. Now I don't know if I should re-schedule the biopsy and eat gluten for 4-6 weeks, but I ate some today and already feel sick. I really don't want to disrupt my life again, I have been feeling sick for a few months now and had been feeling better since i'd improved my iron levels. I'm glad for this forum so I know I'm not alone. I just can't seem to get any real info from any doctors. My original GP told me my iron was fine and that my B12 was low and didnt even test for celiac or ask for any symptoms. Any advice would be great, thanks. |

If you need confirmation to set your mind to it, then I would recommend you reschedule your endoscopy for about 6 weeks out and eat gluten for that entire time and plenty of it. The blood test correlates to how much damage there is, what were your results? _________________ son - high ttg - 4/09, pos. biopsy 5/09

| Thanks, I'm planning to change gastroscopy appointment. But after eating gluten today I feel so sick again with cramps etc and headache and fogginess. I just don't know how I'll make it through two weeks let alone 6 weeks eating gluten for the gastroscopy. As far as I can read my blood test results, my IgA is in normal range, IgA= 2.0 within the normal range of 0-20. But my ANA - speckled result was 160H with the range should be <80. After feeling so sick after just today, I wonder whether I really want a definitive diagnosis. I'm not sure it's worth it for feeling sick all the time. In an ideal world I would like confirmation before eating gf for the rest of my life. I mean if there is a time to test for sure, it's now, before i really start a prolonged gf diet. I wonder that if my reaction is straight away, if they will be able to see the damage anyway after just 2 weeks? Also read that someone on forum said their dr got additional biopsies because they'd being eating gf before test. Thanks for support, just needing to vent. |
