In retrospect, I believe that I have had a minor DH variant that has popped up sporadically over the course of my entire life. The rashes tend to be be very small and localized, but itch like crazy for 12-24 hours prior to their full appearance. This was never diagnosed, and my assumption may be incorrect -- but I doubt it. Has anyone else had a similar experience?

Yes, my experience has been very similar. I had small localized rashes that would appear, mostly on my elbow/forearm for about 15 years before my DH went "full-blown". They would, and still do, itch intensely before they can actually be seen. I can feel the little bumps under my skin at about the time it starts itching. My rashes were always on the left side of my body in those days. When it went to full-blown DH, they became the typical "mirror image" rashes.
